Endovascular surgery
Endovascular embolization is a modern minimally invasive method of surgical treatment of cerebral vascular malformations.
The operation is performed through a single puncture of the artery on the thigh in the groin area. A thin tube-catheter is inserted into the femoral artery, which, under constant X-ray control, advances through the vascular system of the body and is installed in the vessels of the brain directly near the pathological focus of the malformation.
Operation
Angiography of the cerebral vessels is performed, which confirms the correct location of the catheter. After that, an embolization substance is injected through the catheter, which “seals” the pathological vessels of the malformation . Since the structure of the vascular malformation is quite complex, it is not always possible to completely embolize the entire pathological focus.
Sometimes a staged treatment in the form of repeated embolization using different embolizing agents and coils is required. Also, combined treatment is used – microsurgical removal of the malformation after its partial embolization or radiosurgical treatment (irradiation of the malformation).
This approach to treating large AVMs has proven to be the most effective.
Control cerebral angiography
After the operation for embolization of the vascular malformation, it is necessary to perform control cerebral angiography in dynamics (after several months, a year) to detect possible relapses of the disease. All types of surgical treatment of cerebral vascular malformations are used in the neurosurgical department.
