Operations to remove cerebral vascular malformations are among the most complex in neurosurgery. They require the operator to have perfect microsurgical skills and excellent knowledge of neuroanatomy.
Purpose of the operation
Removal of a tangle of pathological vessels to prevent intracranial hemorrhages.

Operation
Microsurgical removal of arteriovenous malformations is performed under general anesthesia using an operating microscope. After trepanation of the appropriate area, staged resection, electrocoagulation and clipping of all pathological vessels and removal of the malformation are performed.
Wake-up anesthesia
If the latter is localized in functionally important areas of the brain (in the area of motor and speech centers), the wake-up anesthesia technique is used: after performing craniotomy, the patient is awakened from anesthesia, and the stage of removing the malformation is performed in consciousness with constant monitoring of motor and speech functions.
After surgery (usually the next day), a computed tomography scan of the brain and cerebral angiography are performed.
